If you are registering your student for the 2024-2025 (Current) school year, please contact Marianne Prete at (440)204-1723 for registration instructions.
Welcome to Vermilion Local Schools!
All families are invited to register online. You will be asked to upload the required documents and complete all the registration forms.
All registrations are made through FinalForms. Below, you'll be able to learn how to create a parent account and register a student.
Step 1: Set up a Parent Account in FinalForms
Go to:
vermilion-oh.finalforms.com
Locate the parent icon and click NEW ACCOUNT.
Type your YOUR NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.
NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, then email
[email protected]
.
Check your email for an ACCOUNT CONFIRMATION EMAIL from FinalForms. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
Click REGISTER STUDENT for your first child (see instructions below for registering a student).
Step 2: Register a Student In FinalForms
What information will I need?
This process will take at least 45 minutes. Please take a moment to gather the following necessary information at the time of registration to expedite the process:
Basic medical history and health information.
Contact information for doctors, dentists, and medical specialists.
Hospital preference and contact information.
How do I register my first student?
Create a parent account using the steps above.
Go to:
vermilion-oh.finalforms.com
Locate the parent icon and click LOGIN.
Locate and click the ADD STUDENT button.
Type in the LEGAL NAME and other required information. Then, click CREATE STUDENT.
Complete each form and sign your full name (i.e., ‘Jonathan Smith’) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the following form.
When all forms are complete, you will receive a confirmation email stating that all forms are completed and signed.
You may now begin to upload and submit all required documents using the instructions below.
How Do I Register Additional Students?
Click MY STUDENTS. Then, repeat the steps above for each additional student.
Step 3: Upload Required Documentation
Custodial parents/guardians must now upload all required documents for enrollment AFTER completing STEPS 1-2 ABOVE.
Copies of the following documentation will need to be uploaded into FinalForms.
Student’s Birth Certificate
Immunization Records
Parent/Guardian Photo ID (Valid)

current (last 30 days) proofs of your residency are listed below.
Primary proof of residence - Acceptable documents:
include a signed rental agreement, mortgage statement, deed, purchase agreement, or construction agreement. This document must list you as the homeowner or leaser.
Secondary proof of residence -
Acceptable documents:
Gas/Water/Electric Bill only
W2
Voter Registration
Property Tax Bill
Pay Stub
Bank Statement
Homeowner insurance or car insurance
If applicable, please be prepared to provide any of the following:
Custody documents - Must be signed and court stamped
Individualized Education Plan (IEP)
Important Documents
Request for a Bus Change
Administration of Medication at School
Residency Affidavit
Immunization Summary for Schools
Grandparent Power of Attorney Form
Grandparent Caretaker Authorization Affidavit
