- Nord Center Both adults and children new to services can walk in Monday-Thursday 8:00am-2:00pm, Friday 8:00am-1:00pm and they will be seen for an intake. We will no longer be scheduling any appointments for intake assessments. Same Day Access is available at our main office, 6140 South Broadway, Lorain, Ohio and clients should expect to spend about two hours for the whole process.
- Clients must bring:
· ID, Insurance card, including Medicaid and Medicare, if insured
· If they do not have insurance, please bring proof of current income, including income of any spouse. Proof of income includes pay stubs and benefit statements. We must have this information when they come in to determine the cost of service.
· If bringing a child - Please come and stay with the child, custody paperwork will be required for the child to be seen unless both parents come/ If single parent never married, no custody paperwork needed.
· If legal guardian of an adult, guardianship paperwork will be required.
